New Student Registration opens February 20, 2024.

Please follow the instructions and complete the forms below.

Registration Fee (per child): $250.00
                                                 Due at time of Registration and is Non-Refundable.
 
What to bring with you:         *Birth Certificate, Social Security Card, and Baptismal Certificate
                                                   * Must have when submitting Registration *
                                                   **Florida Immunization Form (HRS 680)
                                                   **Florida Physical Examination Form (HRS 3040, both sides)
                                                    **Must have by August 1st or will not be able
                                                        to attend school**

1.  Complete Application Form online

2. Complete the FINANCIAL COMMITMENT AGREEMENT
    
3.  Complete the TUITION PAYMENT PREFERENCE FORM

4.  Complete the FACTS payment set-up online
           https://online.factsmgt.com/signin/3T36S
    
5.  Read and sign the following forms PROBATION PERIOD POLICY,  
      VOLUNTEER AGREEMENT, SACRAMENTAL INFORMATION

6.  Attach cash or a check (made out to Saint Mary Catholic School)
     - The Non-Refundable Registration Fee is $250 per students.              

 

PLEASE BE AWARE THAT REGISTRATION IS NOT COMPLETE UNTIL STEPS
1 T0 6 ARE COMPLETE AND RETURNED TO OFFICE. 

ONCE REGISTRATION IS COMPLETE YOU WILL BE NOTIFIED WHEN YOUR STUDENT(S) IS ACCEPTED
